[CRUSH-all] Phoenix info and forms

Hi all
We have a packet of paper with information and forms that each family with a
student participating in the Arizona Regional next week needs to receive,
read and fill out, and return. It will be available for pick up at meetings
each day starting Saturday. Please make sure you get this done. Pasted below
is the information from the first page of that packet, which provides the
main information about costs, times, and places.

Team members, mentors, and families:
Next week CRUSH1011 travels to Phoenix for the FIRST Arizona Regional
competition. It promises to be a fun and exciting time, and a culmination of
our efforts designing and building Kareem. Please read the attached pages
carefully and follow instructions. Help us make this a great trip for all of
us and a good representation of our team and school.
WHEN: We'll leave immediately after school on Wednesday March 8 (be ready by
3:30 pm) and return to school around 6 pm Saturday March 11.
WHERE: The competition is at Veterans Memorial Coliseum. Attached pages give
complete information about location and schedule. It is open to the public
and free except for parking, so if you're not traveling with the team, feel
free to meet us there. Our lodging is at a nearby Best Western, and
information about that hotel is also attached.
STUDENT COST: The per person cost for the trip is $210, including
transportation, lodging, and food. Students may want to bring money for
incidental personal purchases (souvenirs etc) at their discretion. Team
members can apply CRUSH hours to their cost at a rate of $1.75 per hour. If
you have fewer than 120 hours, please make this calculation:
(120 - your hours) * 1.75 = amount you owe
Please bring a check payable to Sonoran Science Academy in this amount. You
must make this payment before we leave on Wednesday. Note that the Las Vegas
trip at the end of March will be more expensive. If you have remaining CRUSH
hours after the Phoenix trip you can apply them to the Las Vegas trip.
MENTORS have been advised separately about their costs.
NON-MENTOR family members or friends who have made arrangements to travel
with the team will owe the full amount of $210 per person.
PERMISSION FORMS: Included in this packet are two forms that must be
completed and returned as soon as possible, along with your payment if
applicable. One is the FIRST consent/release form, the other is the SSA
permission form. Please find them in this packet, complete and return them
promptly.
HOTEL SLEEPING ARRANGEMENTS: In the case of family groups of 3 or 4 we will
attempt to room them together. Students traveling without adult family
members will be roomed no more than four to a room and girls and boys
separately. You may want to consider if you need to bring a sleeping bag.
SAFETY: you are responsible for knowing and following safe procedures,
including:
a.. Safety glasses whenever you are in the pit area
b.. No loose clothing
c.. No open toe shoes
d.. Proper handling and storage of tools, parts, extension cords, etc
BEHAVIOR: Sensible and mature behavior is expected of all team members.
While the team is at the competition you must stay in the building or in the
picnic areas immediately outside. Generally you are expected to either be
helping in the pit or watching the competition. You may not leave the
campus. In addition, you must be available when the team needs your support.
This means when you are not with the team, you either have a cell phone that
we know, or you are with someone who does.
